Thank you for your interest in becoming an ACM Trojan.  Please see the participation information below.  You must have all required forms listed below completed and returned to the Athletic Director or head coach prior to the beginning of your athletic season. Please understand that you will not be permitted to participate with your team until all forms are on file.

• Student-Athlete Health History Questionnaire Form
• Pre-Participation Physical Evaluation Physician Form (to be completed and signed by your primary physician)

• NJCAA Region XX Communicable Disease Waiver
• Emergency Contact/Insurance Form
• Assumption of Risk/Release
• Insurance and Treatment Consent Acknowledgement

• FERPA Release of Information
• NJCAA Sportsmanship and Code of Conduct
